New Jersey Exclusion List (Medicaid and Treasury)

The New Jersey Exclusion List searches both lists New Jersey requires Medicaid providers and contractors to
check every month under NJ Division of Medical Assistance and Health Services Newsletter Vol. 33 No. 02
(Section 6032 of the federal Deficit Reduction Act):
- State of New Jersey Debarment List: providers the Office of the State Comptroller,
Medicaid Fraud Division, has found ineligible to take part in Medicaid and NJ FamilyCare
(N.J.S.A. 30:4D-17, N.J.A.C. 10:49-11).
- N.J. Treasurer's Exclusions Database: Treasury's Consolidated Debarment Report of
individuals and firms debarred, suspended or disqualified under Executive Orders #34 (1976) and #189 (1988),
in five categories: Medical, Professional, Vendor, Construction, and Solid/Hazardous Waste.
Every match shows which of the two lists it came from, and a person on both lists appears once for each. This
list replaces the separate NJ Debarment and NJ Treasury Debarment lists; reports run on those lists are
unchanged and still available.
- Why it matters: Payments for items or services furnished, ordered, or managed by an
excluded or debarred provider or contractor are recoupable and may trigger civil penalties.
- What to verify: Individual or business name, NPI, date of birth (Medicaid list), last known
address, action, category, reason code, debarring department, and effective/expiration dates.
- Update cadence: The Medicaid list is published monthly; Treasury updates as actions occur.
Screen before hiring or contracting and at least monthly thereafter.
- Compliance tips:
- Use alongside OIG LEIE and SAM/EPLS for full federal and state coverage.
- Retain dated evidence of each monthly search for the period required by your policy (commonly 7 years).
- Treat a match on either list as cause for immediate review.
